How to Apply:
There is no formal application format. However, each grant request should include the following:
1. Full name, mailing address, telephone and fax number, e-mail address, name of contact person, and website of organization.
2. A copy of the organization's 501(c)(3) determination letter from the IRS.
3. A copy of the annual budget of the organization.
4. A brief summary of the history, mission, and organizational structure (including names of board of directors) of the applicant organization.
5. The amount of the grant requested and a description of how the grant would be used.
please note:
All grant requests should be sent by e-mail to this address. Requests should be sent between June 1 and July 31. The foundation will award grants between October and December of that year. A recipient organization will be deemed to consent to being listed on our Grant History Page and elsewhere on the website.
